Pain management has improved in the past few decades. Opioid analgesics have become the mainstay in the treatment of cancer pain whilst inter-disciplinary pain management programmes are the generally accepted approach to chronic pain of non-malignant origin. Recently some pain specialists have advocated the use of opioids in the long-term management of non-cancer pain. This has raised some fundamental questions about the purpose of pain management. Is it best to opt for maximum pain relief and comfort, or should one emphasise (...) function and activity as higher priorities? Will the use of opioids create more autonomy for pain sufferers or will this add handicaps to lives which are already limited? Until more clinical outcome data are available we advocate caution in the use of opioid analgesia. Such caution can, and does, raise questions about the rights of the patient and the rights of the prescriber in a context where the facts do not point to a clear course of action. (shrink)

We propose an action-oriented understanding of emotion. Emotions are modifications of a basic form of goal-oriented striving characteristic of human life. They are appetitive orientations: pursuits of the good, avoidances of the bad. Thus, emotions are not truly distinct from, let alone opposed to, actions -- as erroneously suggested by the classical understanding of emotions as 'passions'. In the present paper, we will outline and defend this broadly enactive approach and motivate its main claims. Our proposal gains plausibility from a (...) literature- and interview-based investigation of emotional changes characteristic of clinical depression. Much narrative evidence from patient reports points towards the conclusion that many of those changes might result from a catastrophic alteration of the basic form of goal-pursuit at the root of human emotionality. The experience of profound depression could in this respect be a kind of inverted image of non-pathological emotionality--a highly unnatural passivity, giving rise to a profound -- and quite horrifying -- sense of incapacity. Indeed embracing this tradition is, said Smith, an act of rejoining the human race. Our current forms of organized religion offer us ritualized expressions of this tradition, yet often fail to provide us with transpersonal growth; it is this transpersonal growth that reconnects us with the source of religion. This study explores how distributing the controls of a video game among multiple players affects the sociality and engagement experienced in game play. A video game was developed in which the distribution of game controls among the players could be varied, thereby affecting the abilities of the individual players to control the game. An experiment was set up in which eight groups of three players were asked to play the video game while the distribution of the game controls was increased (...) in three steps. After each playing session, the players’ experiences of sociality and engagement were assessed using questionnaires. The results showed that distributing game control among the players increased the level of experienced sociality and reduced the level of experienced control. The game in which the controls were partly distributed led to the highest levels of experienced engagement, because the game allowed social play while still giving the players a sense of autonomy. Research within the H2020 PROGRESSIVE project has identified good practices in user co-production strategies and methodologies. Early findings from research in the PROGRESSIVE project were shared with relevant stakeholders outside the consortium for consultation and review. The outcomes of that initial investigation highlighted the need to focus on the objectives, processes, and methods used in user and older people co-production. This guide adapts these insights and makes them relevant specifically for standardisation in ICT for active and healthy ageing. This guide (...) was approved by representatives of the PROGRESSIVE project on 22 February 2018. The consortium has requested comments from interested stakeholders in an enquiry from 1 March to 30 April 2018. The PROGRESSIVE guide was approved on 5 June 2018. (shrink)

Inspired by Castañeda (1966, 1968), Perry (1979) and Lewis (1979) showed that a specific variety of singular thoughts, thoughts about oneself “as oneself” – de se thoughts, as Lewis called them – raise special issues, and they advanced rival accounts. Their suggestive examples raise the problem of de se thought – to wit, how to characterize it so as to give an accurate account of the data, tracing its relations to singular thoughts in general. After rehearsing the main tenets of (...) two contrasting accounts – a Lewisian one and a Perrian one – in the first section of this paper, in the second I will present a proposal of my own, which is a specific elaboration of the Perrian account. In the first section I will indicate some weaknesses of Perry’s presentation of his view; the proposal I will articulate in the second overcomes them. I will conclude with a brief discussion of reasons for preferring one or another account, in particular regarding the issue of the communication of de se thoughts. (shrink)

Vacuolar ATPases (V‐ATPases, V1Vo‐ATPases) are rotary motor proton pumps that acidify intracellular compartments, and, when localized to the plasma membrane, the extracellular space. V‐ATPase is regulated by a unique process referred to as reversible disassembly, wherein V1‐ATPase disengages from Vo proton channel in response to diverse environmental signals. Whereas the disassembly step of this process is ATP dependent, the (re)assembly step is not, but requires the action of a heterotrimeric chaperone referred to as the RAVE complex. Recently, an alternative pathway (...) of holoenzyme disassembly was discovered that involves binding of Oxidation Resistance 1 (Oxr1p), a poorly characterized protein implicated in oxidative stress response. Unlike conventional reversible disassembly, which depends on enzyme activity, Oxr1p induced dissociation can occur in absence of ATP. Yeast Oxr1p belongs to the family of TLDc domain containing proteins that are conserved from yeast to mammals, and have been implicated in V‐ATPase function in a variety of tissues. This brief perspective summarizes what we know about the molecular mechanisms governing both reversible (ATP dependent) and Oxr1p driven (ATP independent) V‐ATPase dissociation into autoinhibited V1 and Vo subcomplexes. Recently, some philosophers have argued that we should take quantification of any (finite) order to be a legitimate and irreducible, sui generis kind of quantification. In particular, they hold that a semantic theory for higher-order quantification must itself be couched in higher-order terms. Øystein Linnebo has criticized such views on the grounds that they are committed to general claims about the semantic values of expressions that are by their own lights inexpressible. I show that Linnebo’s objection rests on the assumption (...) of a notion of semantic value or contribution which both applies to expressions of any order, and picks out, for each expression, an extra-linguistic correlate of that expression. I go on to argue that higher-orderists can plausibly reject this assumption, by means of a hierarchy of notions they can use to describe the extra-lingustic correlates of expressions of different orders. This chapter explores how LEGO compare to the metaphysics of the real, actual world—our universe. LEGO worlds and real worlds at least differ in how many there are: there are many LEGO worlds, but only one real world. According to David Lewis, there are worlds in which people were saved from entering the dark ages. There are worlds where they have a billion bricks at their disposal. But there are also worlds where LEGO has never been invented. On Lewis's view, (...) other worlds are just as real as the actual world. Lewis's plausible thesis of Humean supervenience need not be combined with his extravagant claim that there is a real world for any possible arrangement of the fundamental building blocks. A pervasive and obvious phenomenon of our world, is absent in a LEGO world. Things change—cars move, babies grow, coffee goes cold, leaves turn brown. (shrink)

A conjunction of two hypotheses may provide a better explanation than either one of them individually, even if each already provides a good explanation on its own. An appropriate measure of explanatory power should reflect this, but none of the measures discussed in the literature do so because they only consider how much an explanatory hypothesis reduces our surprise at the evidence – which is problematic. This chapter introduces and defends a class of coherentist measures of explanatory power, and shows (...) that one particular such measure that combines different intuitions underlying the epistemological notion of coherence overcomes the aforementioned problems of surprise reduction measures. We compare various notions of algorithmic randomness. First we consider relativized randomness. A set is n-random if it is Martin-Löf random relative to ∅. We show that a set is 2-random if and only if there is a constant c such that infinitely many initial segments x of the set are c-incompressible: C ≥ |x|-c. The ‘only if' direction was obtained independently by Joseph Miller. This characterization can be extended to the case of time-bounded C-complexity. Next we prove some results (...) on lowness. Among other things, we characterize the 2-random sets as those 1-random sets that are low for Chaitin's Ω. Also, 2-random sets form minimal pairs with 2-generic sets. The r.e. low for Ω sets coincide with the r.e. K-trivial ones. Finally we show that the notions of Martin-Löf randomness, recursive randomness, and Schnorr randomness can be separated in every high degree while the same notions coincide in every non-high degree. We make some remarks about hyperimmune-free and PA-complete degrees. (shrink)

The current study assessed potential relationships among childhood wayfinding experience, navigational style, and adult wayfinding anxiety in the Faroe Islands. The Faroe Islands are of interest because they have an unusual geography that may promote the use of an orientational style of navigation (e.g., use of cardinal directions). Faroese adults completed questionnaires assessing (1) their permitted childhood range sizes, (2) the types of navigational strategies they use, and (3) the amount of anxiety they experience when navigating in adulthood. Males had (...) more childhood wayfinding experience, used the orientation strategy at a higher rate, and showed lower levels of wayfinding anxiety. When compared with other cultures, both Faroese women and men appear to embrace orientation strategies at an unusually high rate. Childhood experience was not conclusively linked to later wayfinding anxiety. However, the current findings raise the possibility that children who have particularly small ranges in childhood may be especially anxious when navigating in adulthood. (shrink)

Coherence considerations play an important role in science and in everyday reasoning. However, it is unclear what exactly is meant by coherence of information and why we prefer more coherent information over less coherent information. To answer these questions, we first explore how to explicate the dazzling notion of ``coherence'' and how to measure the coherence of an information set. To do so, we critique prima facie plausible proposals that incorporate normative principles such as ``Agreement'' or ``Dependence'' and then argue (...) that the coherence of an information set is best understood as an indicator of the truth of the set under certain conditions. Using computer simulations, we then show that a new probabilistic measure of coherence that combines aspects of the two principles above, but without strictly satisfying either principle, performs particularly well in this regard. This article investigates the notion that there is an asymmetry in openness between the past and the future. The following questions are considered: How exactly is this asymmetry in openness to be understood? What is the relation between an open future and various ontological views about the future? Is an open future a branching future? What is the relation (...) between an open future and the question of whether contingent statements about the future are true or false? Recent studies indicate that adapting common components of universal healing practices increases the effectiveness of multicultural therapies, especially incorporating initial and reformulated myths. The Socratic method, part of an original philosophical process directed toward therapeutic goals, has long been instrumental to many psychotherapies, but limited in application to dialectical discourse. Through a rediscovery and clarification of the original integrated Socratic-Platonic method inclusive of mythmaking as well as systematic questioning, the authors argue that this new, more comprehensive model provides a foundation (...) for increasing the effectiveness of multicultural psychotherapies. Conditionals pervade every aspect of our thinking, from the mundane and everyday such as ‘if you eat too much cheese, you will have nightmares’ to the most fundamental concerns as in ‘if global warming isn’t halted, sea levels will rise dramatically’. Many decades of research have focussed on the semantics of conditionals and how people reason from conditionals in everyday life. Here it has been rather overlooked how we come to such conditionals in the first place. In many cases, they (...) are learned through testimony: someone warns us about the ill-effects of cheese. Any full account of the conditional must consequently incorporate such learning. Here, we provide a new formal account of belief change in response to a testimonial conditional. (shrink)

There is currently disagreement about whether the phenomenon of first-person, or de se, thought motivates a move towards special kinds of contents. Some take the conclusion that traditional propositions are unable to serve as the content of de se belief to be old news, successfully argued for in a number of influential works several decades ago.1 Recently, some philosophers have challenged the view that there exist uniquely de se contents, claiming that most of the philosophical community has been under the (...) grip of an attractive but unmotivated myth.2 At the very least, this latter group has brought into question the arguments in favor of positing special kinds of content for de se belief; I think they have successfully shown that these arguments are not as conclusive, or fully articulated, as many have taken them to be. In this paper I will address these challenges directly and I will present and defend an argument for the conclusion that the phenomenon of de se thought does indeed motivate the move to a special kind of content, content that is uniquely de se. First, I characterize a notion of de se belief that is neutral with respect to friends and foes of uniquely de se content. I then argue for a determination thesis relating de se belief to belief content: that there is no difference in de se belief without a difference in belief content. I argue that various proposals for rejecting this determination thesis are unsuccessful. In the last part of the paper, I employ this determination thesis to argue for the existence of a type of belief content that is uniquely de se. (shrink)

Models are of central importance in many scientific contexts. The centrality of models such as inflationary models in cosmology, general-circulation models of the global climate, the double-helix model of DNA, evolutionary models in biology, agent-based models in the social sciences, and general-equilibrium models of markets in their respective domains is a case in point (the Other Internet Resources section at the end of this entry contains links to online resources that discuss these models). Scientists spend significant amounts of time building, (...) testing, comparing, and revising models, and much journal space is dedicated to interpreting and discussing the implications of models. Spatial experience in childhood is a factor in the development of spatial abilities. In this study, we assessed whether American and Faroese participants’ (N = 246, Mage = 19.31 years, 151 females) early spatial experience and adult spatial outcomes differed by gender and culture, and if early experience was related to adult performance and behavior. Participants completed retrospective reports on their childhood spatial experience, both large-scale (permitted childhood range size) and small-scale (Lego play). They also completed assessments of their current (...) large-scale spatial behavior (navigational strategy) and small-scale ability (mental rotation task, MRT). We replicated earlier results showing better MRT performance among males and more reliance by males on orientation navigational strategies, although males and females reported similar ranges as children. However, there were cross-cultural differences, with Faroese having larger childhood ranges, less reliance on route strategies, better MRT scores, and a smaller gender difference in MRT. Larger permitted childhood ranges were associated with reduced use of route strategies for navigation in adulthood, and greater Lego play in childhood predicted better MRT performance as adults. There was also some evidence supporting relationships across spatial scales, with more Lego play predicting an orientation style of navigation and larger childhood ranges predicting better performance on the MRT, although the latter was not independent of country. In sum, we observed an association in both cultures between large-scale childhood experience and large-scale behavior in adulthood, small-scale experience in childhood and adult small-scale performance, and some associations between experience and behavior across spatial scales. The relation of global supervenience is widely appealed to in philosophy. In slogan form, it is explained as follows: a class of properties A supervenes on a class of properties B if no two worlds differ in the distribution of A-properties without differing in the distribution of B-properties. It turns out, though, that there are several ways to cash out that slogan. Three different proposals have been discussed in the literature. In this paper, I argue that none of them is (...) adequate.
